LCC CAPITAL EXPENDITURE REQUEST


SUPPLEMENTAL APPLICATION QUESTIONS


A capital expenditure is for an item(s) that have a life expectancy of more than three years and a monetary value of more than $500. Examples include equipment, such as lighting or sound equipment, new construction or the renovation of a cultural facility. Please refer to the LCC Program Regulations & Guidelines for additional information.

If you are applying for a capital expenditure project, you must answer the questions below that pertain to your particular situation, in addition to the LCC Application Form. Answers must be typed and may not exceed one page.

QUESTIONS FOR LCC CAPITAL EXPENDITURE REQUESTS


1. How will the equipment be incorporated into the organization's programming/offerings?

2. Can you demonstrate a 2:1 match? What are the sources of these matching funds?

3. If making a purchase, who will own the equipment?

4. Who will have access to the equipment?

5. What is the long-term public benefit of this request and how will this be evaluated? Who specifically is benefiting from the expenditure?

6. Do you have plans for future maintenance of the equipment? Who will be responsible for maintenance costs?

7. What provisions/arrangements has your organization made for the equipment in the event that you can no longer continue your operations?
